Record requests are processed within 48 hours, excluding weekends and holidays. Photo identification issued by a state or federal agency is required. A request form can be obtained in person or can be mailed. If you are unable to access your medical records using My Health Record or you need a paper copy, you will need to complete a Records Request form and return it to the Health Information Management department. Only you can decide who has access to your medical history, which ensures your privacy. You can also use it to help manage the health of someone you care for, with their permission.
